Job Description
INTRODUCTION:
The Case Manager evaluates members in assigned caseload, develops and implements Day Habitation Service Plans for members and program objectives, and acts as a resource for members, families, team members and program staff. Duties will also include direct supervision and training of members and participation in their program activities, within community settings or within the facility, behavioral plan implementation, direct care responsibilities and others. Serves as member of the interdisciplinary team in the coordination of therapeutic services.
PRIMARY DUTIES:
-Develop the appropriate need assessments and goals of Day Hab Service Plans and methods for implementation; support member’s choices in program planning. Participate and negotiate in case conferences and support planning. Act as a resource to members, families, referral and residential staff. Maintain positive relationship with referral agencies.
-Assist in-take and orientation process for new participants. Evaluate members to determine service needs with emphasis on preferences expressed by individual.
-Ensures implementation of members DHSP objectives & corresponding data collection. Accurately records all data for established goals, objective and behavioral plans to determine progress & needs.
-Organize and update individual records; complete required paperwork in a timely and accurate manner.
-Participate in Case Reviews with proper documentation of members progress for assigned caseload
-Trains program staff on the DHSP and goals and objectives. Reinforce a team approach with open communication, support and instruction.
-Consult with Occupational, Speech, Physical Therapists and Behavioral Analysts for therapeutic assessments of members and develop therapeutic goals and objectives from the assessments.
-Counsel members/make referrals and provide follow-up for additional services as indicated by member needs.
-Assist in teaching new skills to members; facilitate individual or group activities in areas of self advocacy, social/developmental skill training, community integration & others determined by DHSP. Maintain knowledge of members learning styles, strengths & needs to teach skills effectively. Assist program staff in carrying out personal care duties for members, including, but not limited to transfers from wheelchairs, toileting, showering, lifting and transfers to and from vehicles.
-Participate in the development of Community Inclusion activities for targeted skill development areas and other areas as indicated by therapeutic services
-Crisis intervention including: consistently carrying out behavior plans; intervening in crisis situations, as needed; proper implementation of Behavior Management & Emergency Restraint interventions; accurate & timely completion of incident reports & other related paperwork.
-Be aware of Human Rights & Sexuality policies, & monitor that consumers’ rights are upheld; act as a role model by displaying proper dress, language & a positive attitude.
-Ensure that Health & Safety policies & procedures are implemented; be familiar with evacuation routes, ensure safe & efficient evacuations during drills & actual emergencies; be aware of fire safety & prevention procedures; keep aisles & means of egress clean & accessible; report unsafe situations; monitor consumers during lunch, breaks & cab duty; & safely transport members to community sites. Keep program clean & organized by performing light housekeeping;. Always use safe lifting & transferring techniques.
-Participate in Quality Assurance and Case Records Committees’ survey processes; and contribute to the Program Evaluation Process, as requested.
-Assist in staff scheduling and rotation of back-up on call support
-Maintaining petty cash and funds allotted to the program per fiscal policy and procedures
QUALIFICATIONS: Bachelor’s Degree in Behavioral Sciences, Psychology, Rehabilitation Education, or related field with two years experience in Human Services; or Masters Degree in Human Services and one year experience in the Rehabilitation field
ADDITIONAL QUALIFICATIONS: First Aid and CPR Certification, Knowledge and use of behavior modification techniques, Community Inclusion, and Principles of Normalization, experience in providing services to individuals with Mental Retardation, Developmental Disabilities, and elderly individuals with developmental disabilities. Strong writing skills
PAY RANGE: $33,000 - $38,000 Annually
