Our Providers are responsible for providing health care to patients enrolled in the CareSight Program in their home. The Provider leads a care team that consists of Registered Nurses and Care Coordinators and works closely with the patient's primary care physician.Job Responsibilities:Delivers cost-effective, high-quality care to assigned CareSight patientsCompletes comprehensive history and physical examination on all enrolled patients in an in-home visitCompletes a 360 nursing assessment on selected patients in an in-home visitConducts a Patient Activation Measure (TM) (PAM) initially and periodically during patient's enrollment in CareSightCollaborates with RNs and patient to determine patient's risk of adverse health outcomesAdjusts the patient's risk level as appropriate based on assessmentsCompletes proactive, in-home visits on all intensive level patients at least monthlyCompletes proactive, in-home visits on all core level patients quarterlyCompletes unplanned, urgent, and/or emergent in-home visits as needed to meet patient needsAdjusts visit frequency and schedule based upon patient needsResponds to calls from the patient, caregiver, CareSight RNs, etcAddresses Advance Care Planning proactively; identifies surrogate decision maker (MPOA, Health Proxy, etc)Manages patient's medical care as appropriate including:Orders and monitors of diagnostics including laboratory studies, radiological studies, etc.Refers to appropriate specialists or community services, ie Home Health, DME, community resourcesAddresses recommended preventive and quality measures; acts to close potential care gapsMonitors chronic conditions to minimize exacerbations or treat (in place if clinically appropriate)Identifies, assesses, diagnoses, treats acute changes of condition (in place if clinically appropriate)Manages medication therapy effectively:Reviews and reconciles medications each visitAvoids high-risk medicationsPrescribes medication, adjusts dosages, discontinues medications as appropriateSimplifies medication regimen for improved adherenceAdministers selected medications in the home (if clinically safe to do so)Collaborates with RNs in the development of an integrated care plan that identifies key patient problems, goals of care, barriers to meeting goals, interventions, activation level, and patient self-management activitiesCommunicates effectively with entire care team including the patient's community primary care providerProvides effective patient education using the Teach Back techniqueDocuments all patient encounters (in-home and telephonic) per documentation standardsParticipates in clinical case conferences as requiredParticipates in on-call coverage for patient care Requirements:Holds active, unencumbered license in state of practiceCredentialed in adult, family, or geriatric careHas active DEA licenseExperience in care of adult, chronically ill patientsTwo or more years of practice experience preferredDriver's license without restrictions and good driving recordWorking knowledge of computers and ability to document effectively and efficiently in an electronic system
