Windows Database App (Cranston) - Outsource.com (Cranston, RI) in Providence, Rhode Island For Sale

I need a simple standalone database app that will let us scan in an invoice form from a twain compliant scanner (or we can pre-scan and import jpgs if that's easier) and have a screen pop up and let's us view the image and enter in hand written info from the form into individual fields. There will be about 15 fields. The database must be searchable by at least date, last name, phone, VIN (automotive) or product. One field (Product sold) will have to be from a pull down list and we must have the ability to add/remove entries for that pull down. IDEALLY we would also like an iOS companion app that would let us search the database or have it where it will work with an already existing iOS app. We will store the saved entries locally but would mirror them to Dropbox or other similar service if it helps make the iOS companion app possible within our budget. We will provide the list of fields. If you can make it so we can add/remove fields that's a big bonus but I have no idea how that would affect the existing database if fields suddenly changed. If the saved data could be stored in plain text csv that would be nice too for future-proofing.
